Your dental coverage is based on the policy you or your employer buy from an insurance company. You or your employer make the decision about the type of policy based primarily on financial considerations. The less you or your employer paid for your insurance, the less benefits you will receive.

If you want to know what is covered, you must read the fine print of your contract. Many plans tell their insured that they will be covered “up to 80% or up to 100%” but do not clearly specify the plan fee schedule, annual maximum, waiting periods, or other limitations. Things do vary from carrier to carrier, but we have found that a general rule is that most plans tend to cover only 35%-65% of major services.

"Approved or Accepted Amount" is a term developed by the insurance carrier industry to reflect "average charges" from specific dentists in designated geographic localities. This amount noted on the explanation of benefits does not necessarily reflect the charges for your treatment.

Please watch for the explanation of benefits to arrive from your insurance company. Some, and perhaps all, of the services provided may be non-covered services and not considered reasonable and customary under the terms of your insurance policy. Our practice is committed to providing the best treatment for our patients and we charge what is the Usual and Customary for our area.

You are responsible for payment regardless of any insurance companies arbitrary determination of usual and customary rates. Your complete insurance information must be presented before services are provided. You will need information for both primary and secondary coverage (should you have dual coverage). Insurance claims cannot be backdated. Our office verifies benefits prior to your appointment or to billing the insurance.

If you have any questions regarding your insurance, we ask that you contact your company regarding the specifics and details of the plan it is conducting in your behalf. Because you are the insured, you have an established relationship with your insurance carrier. Therefore, you must understand that your contract is between you and your insurance provider. Your contract for dental treatment is made directly with us.
